New User Setup

Do not use this form until your service is installed and operating properly. This form is used to setup your customer account, pay for installation and setup automatic monthly subscriptions. Shelby Telecom uses PayPal to process payments.

If you already have a PayPal Account you can have your monthly subscription deducted from your checking account monthly, effective on the day of the month that you process the first payment.

If you do not have a PayPal Account you will be required to provide credit card information on the following screens. Your payment is processed on PayPal Secure Servers and all information is encrypted. You ARE NOT required to setup a PayPal Account to pay for installation and service.

A monthly charge of $40.00 is required to maintain service. Currently Shelby Telecom provides no other means of payment than through a checking account draft or credit card payment through PayPal. Your payment will be charged on a monthly basis beginning on the date you process the first payment. If your payment fails for any reason the PayPal system will attempt the payment again. If it fails again, your subscription will be cancelled. A reconnection fee of $40.00 and all charges due must be paid before service is restored.

If you cancel your service for any reason, you agree to return all installed equipment to Shelby Telecom within 30 days of cancellation. Failure to return equipment may result in legal action being taken to recover costs associated with replacing the equipment. Installed equipment remains the property of Shelby Telecom and users are not allowed to hack, damage, reverse engineer or otherwise alter installed equipment. IP address schemes assigned to the network are designed to protect the integrity of the network and should not be altered.
